The Deficiencies are what 'Owner' needs to attended to; as in MUST attend to. 
Fire and Building Codes require this AND your insurance company will also require same.

Head box will be about $25.  Head Wrench: Depends on the heads you have installed. $165 to $225 estimate.

Heads: Tyco 3596 Residential LFII Concealers, with escutcheons $20.00 each. For a 6-head box: need 6 heads.

This data is supposed to be on the drawings left with 'Owner' at occupancy, by Builder, AND the Data is also supposed to also be on the inside of the Head box.

I can only guess that the test he did, showed that it was not the required Utah State Code requisite antifreeze.
